To: The following entities listed,
YOU ARE HEREBY NOTIFIED, pursuant to sec. 178.09032, sec. 179.0811, sec. 180.1421, sec. 181.1421 or sec. 183.0708 of the Wisconsin Statutes, that the Department has determined that the entity has not paid the annual report fee due the Department, within one year after it was due; or has not delivered to the Department, within one year after it was due, its annual report in condition acceptable for filing, or both; and that the entity will be administratively dissolved unless, by the date indicated below, it corrects each of the aforementioned grounds or satisfies the Department that the grounds do not exist.
