Determination of Grounds for Revocation

To the Entities Named Below:
YOU ARE HEREBY NOTIFIED, pursuant to sec. 178.10102, sec. 179.10102, sec. 180.1531, sec. 181.1531 or sec. 183.09102 of the Wisconsin Statutes, that the Department of Financial Institutions has determined that the entity:

- Has not paid the annual report fee due to the department within four months after it was due; or
- Has not filed with the department, within four months after it was due, its required annual report in a condition acceptable for filing, or both; and

That the entity’s Certificate of Authority/Registration to transact business in Wisconsin will be revoked within 60 days after this notice takes effect.
