What You Need To Get Started

Caution: Make sure you upload the correct version of the Franchise Disclosure Document to avoid additional expense. Only one document can be uploaded per filing, so make sure the entire FDD with exhibits is in one PDF document. Check for errors BEFORE filing.

1. Franchisor Name or File Number: The legal name of the Franchisor or the file number under which your franchise is registered. This number is located on your confirmation e-mail that was sent from DFISecurities@wi.gov.
2. Franchisor's Principal Business Address: The franchisor's principal business address in the U.S. If a foreign franchisor does not have a U.S. business address, use its principal business address in its home jurisdiction.
3. Application States: The states in which this application is or will be shortly on file.
4. Contact Person: The name, address, telephone, facsimile number and email address of the contact person for this registration filing. All communication regarding this filing will be via the e-mail address of the contact person.
5. Contact for Service of Process: The name and address of the franchisor's agent in the state authorized to receive service of process.
6. Franchise Disclosure Document (FDD): An electronic copy of the entire franchise disclosure document in an unsecured .pdf format is required for on-line filing. Make sure you upload the correct document to avoid additional expense. Only one .pdf document can be uploaded per filing, so make sure the entire FDD with exhibits is in one PDF document. Check for errors BEFORE filing.
7. Payment: Payment can be made with a credit card (Visa, MasterCard, Discover, or American Express).  E-Check (electronic check) is accepted.
8. Fee: The Amendment fee is $200.00.
