What You Need To Get Started

Caution: Make sure you upload the correct version of the Franchise Disclosure Document to avoid additional expense. Only one document can be uploaded per filing, so make sure the entire FDD with exhibits is in one PDF document. Check for errors BEFORE filing.

1. Franchise Name: The full legal name (required) of the franchisor.
2. Trade Name: Doing Business As name of the franchisor, if applicable.
3. Franchisor's Principal Business Address: The franchisor's principal business address in the U.S. If a foreign franchisor does not have a U.S. business address, use its principal business address in its home jurisdiction.
4. Application States: The states in which this application is or will be shortly on file.
5. Contact Person: The name, address, telephone, facsimile number and email address of the contact person for this registration filing. All communication regarding this filing will be via the e-mail address of the contact person.
6. Contact for Service of Process: The name and address to contact with any notice, process or pleading served under the uniform consent to service of process.
7. Officer Authorization: The uniform consent to service of process must be authorized by a corporate officer (officer name and title are required).
8. Franchise Disclosure Document (FDD): An electronic copy of the entire franchise disclosure document in an unsecured .pdf format is required for on-line filing. Make sure you upload the correct document to avoid additional expense. Only one .pdf document can be uploaded per filing, so make sure the entire FDD with exhibits is in one PDF document. Check for errors BEFORE filing.
9. Payment: Payment can be made with a credit card (Visa, MasterCard, Discover, or American Express).  E-Check (electronic check) is accepted.
10. Filing Fee: Registration application fee is $400.00.
